Pre-registration with the practice

When you register you need to complete both a Registration form and a medical questionnaire (links below). This is because it can take considerable time for us to receive your medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Online Registration Form
A photo and a proof of address of the person registering needs to be emailed to WFCCG.churchillmedicalcentre@nhs.net once they have completed their online registration.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively, you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
Registration Form
Please try to bring your medical card with you when registering. All new patients will have the opportunity to fill in a health questionnaire, giving details of medical history and lifestyle, and will be offered an appointment for a new patient medical check with the healthcare assistant or practice nurse.
It is essential that you attend this appointment as it provides the practice with important information on your current health needs. We normally register new patients between 12:30 - 14:00, Tuesday to Friday.
If you are unable to attend between these times then be prepared to have a longer wait until someone is available to process your registration.
If you are registering a child under the age of 5 years please bring along their red book at the time of registration so that we can update their childhood Immunisation record .
Change of Address
To help us keep in contact with you, please let us know as soon as possible if your address or telephone number changes either via online, by calling us or by dropping into the practice.

Patient Online Access
What is Patient Online?
Patient Online services will give you the option to:
• Make appointments online
• Order repeat prescriptions online
• View your own medical record online
You will be able to complete these actions using a computer, tablet or smartphone
rather than having to phone or visit your practice.
What are the benefits of online services?
Online services will allow you to book and cancel appointments or request repeat
prescriptions at a time that is convenient to you – day or night. It can also mean not
having to travel to the surgery and can free up phone lines for people without access
to a computer.
Having access to records means, that you could be more in control of your health
and well-being. This is particularly helpful for people who live with a long-term
condition such as Diabetes that needs regular monitoring and frequent prescriptions.
